WHAT IS THE LIKELY DIAGNOSIS?
  • The examination and other diagnostic findings were thought to be consistent with inflammatory choroiditis, and sympathetic ophthalmia vs multiple evanescent white dot syndrome (MEWDS) seemed most likely.
  • As sympathetic ophthalmia was believed to be the most likely diagnosis, the patient was treated with prednisone 60 mg po every morning and Pepcid® 20 mg po, bid.
  • One of the consulting specialists (JDMG) felt the data were consistent with either sympathetic uveitis vs MEWDS but felt MEWDS was more likely. While the delayed choroidal filling in areas of RPE discoloration was compatible with sympathetic uveitis (Ref 14) the wreath-like pattern of early hyperfluorescence at the margin of the lesions was suggestive of MEWDS.
