What are the clinical findings?
The MRI revealed enhancement of right optic nerve on T2 imaging (Figure 5).
What is the differential diagnosis?

Based upon the evaluation, the primary considerations were inflammatory and infiltrative optic neuropathies, including tuberculosis, sarcoidosis, lymphoma, and syphilis. The ESR was 121 mm.

 

What should be the diagnostic approach at this point?
