WHAT WAS THE CLINICAL OUTCOME?

 POSTOPERATIVE OUTCOME AND MANAGEMENT: Postoperatively, the patient’s visual acuity  improved to 20/70 OS and his extraocular motility (EOM) OS improved to 30% movement in all  gazes. The afferent pupillary defect resolved. The intraocular pressure remained 26 mm Hg OS.  Under the direction of the infectious disease service, the vancomycin, 1 g IV, q 12 h; gentamicin, 100  mg IV, q 8 h; and imipenem/cilastatin, 500 mg IV, q 6 h, were substituted for ampicillin/sublactam  and vancomycin.

 The new episode of onset diabetes mellitus, diabetic ketoacidosis, and alcohol abuse was managed by  the medicine service.The patient ultimately suffered a respiratory arrest, which required  resuscitation. This episode was most likely related to autonomic dysfunction combined with a severe  vasovagal episode associated with urinary retention and distended bladder.

 6-WEEKS POSTOPERATIVELY: After 6 weeks of outpatient administration of ampilcillin/sublactam  and vancomycin, the patient's vision improved to 20/40 OS. At 3 months postoperatively, the CT  showed resolution of the sinusitis, clearing of the intraorbital purulent exudates, and clearing of the  soft tissue swelling.
