CASE OF THE MONTH

CASE #20

HOW SHOULD THE PATIENT BE TREATED AT THIS POINT?

Given the copious purulent drainage from the canthotomy site and the severe facial soft tissue   swelling, the patient was urgently taken to the operating department by the orbital and ENT   services for aggressive incision and drainage.
The following procedures were performed:

  • medial and lateral orbitotomy,
  • frontal sinus trephination,
  • left maxillary antrostomy,
  • right nasal polypectomy, and
  • periorbital biopsy with drain placement.
Copious amounts of purulence were expressed from the soft tissues of the left side of the face.
