Pupillary Examination 4 mm round, brisk, no relative afferent pupillary defect OU
   
Extraocular motility Full and straight OU
   
Intraocular Pressure 15 mm Hg OD
  Deferred OS
   
Slit Lamp Examination OD: Unremarkable
  OS: 1 inch nail protruding from sclera 3 mm posterior to the 11:30-o’clock limbal position
  Trace conjunctival injection
  Clear cornea
  Anterior chamber : 2+ cell/flare
  Lens clear
   
Fundus Exam OD: Unremarkable
  OS: Vitreous traction bands in the 5:30-o’clock and the 6-o’clock position.
  Small vitreous hemorrhage
  Retina flat
