WHAT IS THE MOST LIKELY DIAGNOSIS?

Recurrent MEWDS or a related condition (eg, punctate inner choroidopathy) was considered, but sympathetic ophthalmia seemed the most likely diagnosis. The patient was re-treated with steroids with symptomatic improvement. Because the patient became steroid intolerant (cushingoid side effects), the patient was controlled with a tapering dose of prednisone and methotrexate. Her vision has remained 20/20 in the left eye 18 months after presentation.
