CASE #25

 

 
POSTOPERATIVE OUTCOME: DAY 1
  • On the first postoperative day, the patient’s visual acuity was light perception OD with intraocular pressure of 42 mm Hg.
  • Examination was notable for a clear cornea with a closed laceration, an 80% epithelial defect, an aphakic depth anterior chamber, and a full silicone oil fill of the vitreous cavity.
  • The retina was flat on and posterior to the scleral buckle, except in the area of inferotemporal hemorrhage.

TREATMENT

  • Diamox® 250 mg po, qid;
  • Prednisolone acetate 1% every hour while awake;
  • Atropine 1% qid;
  • Ciloxan® qid; and
  • Cosopt® and Alphagan® bid.
  • The patient also received prednisone 75 mg po and Pepcid® 20 mg po daily for 7 days.
