Case:
A 72-year-old African American woman came to the emergency room and reported
a 1-hour history of yellow and purple lights in her right
eye, followed by complete blackening of vision. This painless,
isolated episode was not associated with systemic complaints of temporal
arteritis or other common neurologic complaints. Decreased vision was
noted at the time of examination.
PAST MEDICAL HISTORY: The patient reported a history of hypertension, thyroid resection, with no other significant medical problems. MEDICATIONS: Losartan, thyroid supplement, naproxen ALLERGY: Aspirin EXAMINATION: The patient is a seemingly healthy black woman who appeared younger than her stated age. EMERGENCY DEPARTMENT CARE: In the emergency department, the patient was treated for hypertension due to a systolic blood pressure greater than 200 mm Hg. A noncontrast CT scan of the head was performed, and a neuro-ophthalmologic consultation was obtained. |
