Case: A 59-year-old African American man reported a decrease in vision in his left eye of 2 days’ duration upon arrival at the University Hospital emergency department. This decrease in vision, with associated floaters and flashing lights, occured immediately upon sneezing. A residual gray haze had persisted, which led him to consult the eye clinic. He reported having had similar visual symptoms the previous month, after an episode of vomiting, but these symptoms resolved in about 20 minutes. Before this episode, his visual acuity had been the same in both eyes. The patient reported no medical problems or drug allergies. He had no history of jaw claudication or scalp tenderness. He smokes 1 pack of cigarettes a day and snorts cocaine once or twice a week.
