INITIAL NEURO-OPHTHALMIC EXAMINATION
Visual Acuity    OU: 20/25
   
Ishihara Color Plates  Normal OU
   
Pupillary Examination  — Possible trace relative afferent pupillary defect OD
 — Pupils are equal, round, and reactive to light
   
Lids & Adnexa  1 mm ptosis OS with good levator function (probably
 due to cataract surgery)
   
Slit Lamp Examination  Normal OU
 

 

Ocular Motility  Normal OU
   
Cranial Nerve Examination  Normal
   
Visual Field (confrontation field test)  Unreliable fields
   
Fundus Examination  OU: Normal with a cup-to-disc ratio of 0.2

 

What advice would you give the patient’s physicians?
